OiM cover sheet

Ashley has just brought this to my attention, but you will need to attach a cover sheet to your assessed paper. I have attached a template that you can use. Please remember that the ‘student number’ (OSS number) on these forms is not the University card number (always a seven-figure number beginning with ‘2′) but the number of your student record. It usually consists of from four to six figures and may start with any number. If in doubt, ask in the ISCA general office or leave blank.
Also, make sure that you staple or bind your paper, and attach DVDs (or other material that you may submit with it) securely.
